The fat pads that cushion your eyes are held in place by a thin membrane — the orbital septum. With age, and often genetics, this septum weakens. In the upper eyelid, the levator muscle loses tone and excess skin accumulates, creating a hooded, heavy appearance. In the lower eyelid, fat herniates forward through the weakened septum, producing the characteristic bag beneath the eye.
No cream, filler, or lifestyle change reverses these structural shifts. Eye creams temporarily reduce swelling; fillers can soften shadows. But the displaced tissue remains. This is why patients who have tried everything still look exhausted — the root cause was never addressed.

01 Will my results look natural?
Yes — when planned and executed correctly, eyelid surgery produces results that look completely natural. The goal at Panache is not to change how you look, but to make you look rested and refreshed. 02 What is the difference between upper and lower eyelid surgery?
Upper blepharoplasty removes excess skin and fat from the upper eyelid — correcting drooping, hooding, or a tired appearance caused by skin laxity. It is performed under local anaesthesia or sedation and is one of the quickest, most effective facial procedures available. Lower blepharoplasty removes or repositions fat from beneath the eye — correcting under-eye bags (the puffiness caused by fat prolapse) and tightening loose skin. At Panache, lower blepharoplasty is typically performed via the trans-conjunctival approach (inside the lower eyelid), leaving no external scar. Both procedures can be performed in a single session.
03 How do I know if I need upper or lower eyelid surgery — or both?
Upper eyelid surgery is indicated when there is excess skin causing hooding — skin resting on the lashes or obscuring the upper eyelid platform. It may also be functional if the drooping is significant enough to affect vision. Lower eyelid surgery is indicated when under-eye bags are present (caused by fat prolapse, not just dark circles from pigmentation) or when there is loose skin and a hollow appearance beneath the eye. Many patients have both concerns — combined upper and lower blepharoplasty is very commonly performed in a single session. Dr. Ankit assesses your specific anatomy and recommends the right combination at consultation.
04 What anaesthesia is used for eyelid surgery?
Upper blepharoplasty is most commonly performed under local anaesthesia with or without oral sedation — making it a daycare procedure with minimal downtime. Lower blepharoplasty, particularly when combined with fat repositioning or other procedures, may be performed under intravenous sedation or general anaesthesia. When both upper and lower eyelids are being addressed simultaneously — or when blepharoplasty is combined with a brow lift or fat grafting — general anaesthesia or sedation is typically used for comfort. Dr. Ankit discusses the anaesthesia plan at your consultation based on the scope of the procedure.
05 Will there be visible scars after eyelid surgery?
For lower blepharoplasty, Dr. Ankit uses the trans-conjunctival approach — the incision is made inside the lower eyelid, leaving no external scar whatsoever. For upper blepharoplasty, the incision is placed precisely within the natural upper eyelid crease — which already exists as a natural skin fold. When healed, this scar is virtually imperceptible in the closed eye and completely hidden when the eye is open. Most patients find the scar fully invisible within a few months. Asian eyelid surgery creates the crease and heals similarly — the scar falls within the newly created crease.
06 Can under-eye fillers replace lower eyelid surgery?
Tear trough fillers (typically hyaluronic acid) address the hollow, shadowed area beneath the eye — not the fat protrusion that causes under-eye bags. If the concern is a dark hollow without significant fat prolapse, fillers can produce an excellent result. However, if the lower eyelid has prominent fat bags — the forward protrusion of orbital fat that creates a puffy, heavy appearance — fillers add volume in the wrong area and will not correct the problem. In these cases, surgery (lower blepharoplasty with fat removal or repositioning) is the correct solution. Dr. Ankit will assess which approach is appropriate for your specific anatomy.
07 Are the results of eyelid surgery permanent?
Upper blepharoplasty results are long-lasting — typically 8–12 years or more. The removed skin does not return. However, the natural ageing process continues, and further skin laxity may develop over many years. Lower blepharoplasty (fat removal via the trans-conjunctival approach) is generally considered permanent — the removed fat does not regenerate. Some patients see gradual changes with age as the lower eyelid skin continues to age, but the fat bags themselves do not recur. Overall, blepharoplasty offers one of the best longevity-to-recovery ratios of any facial procedure.
