Under-eye dark circles are a prevalent cosmetic concern, impacting individuals across various age groups and skin types. They can be influenced by multiple factors, and understanding their underlying causes is crucial for determining the most effective treatment options. This article delves into the types of dark circles, including those that are resistant to treatment, and explores where medical interventions like fat grafting, fillers, or excess skin removal can be beneficial.
Types of Dark Circles and Their Causes

Shadowing
These are not true dark circles, and appear due to shadows. Shadowing occurs due to the natural contours and structure of the eye area. The under-eye area may have a hollowed appearance, or there may be a presence of under eye bags, causing shadows to fall and create the illusion of dark circles.
- Treatment: These kind of dark circles can be addressed by under eye fillers (temporary) or Fat Grafting (permanent). By adding volume to the hollowed area, we can reduce the shadow effect and create a smoother, more even appearance.
- Fat Loss (Volume Loss)
Fat loss in the under-eye area can result in a sunken appearance and pronounced dark circles. As we age, the fat pads that cushion the eyes can diminish, leading to deeper shadows and more visible blood vessels.
- Treatment: The best tratment option for these types of dark circles is Fat Grafting, followed by Dermal fillers.
- Hyperpigmentation
Hyperpigmentation around the eyes can result in a darker skin tone due to excess melanin production. This type of dark circle is more common in individuals with darker skin tones. They can be exacerbated by sun exposure, hormonal changes, and genetic predisposition.
- Treatment: Eye creams with ingredients like Vitamin C, hydroquinone (use only under the guidance of a doctor), and retinoids can help lighten hyperpigmented areas. Laser treatments and chemical peels can also be effective in reducing pigmentation and improving skin texture.
- Vascular Causes
Vascular dark circles occur due to the visibility of blood vessels under the thin skin of the under-eye area. This condition can result in a bluish or purplish hue. Thin skin, blood vessel dilation, and poor circulation can contribute to this type of dark circle.
- Treatment: Treatments include retinoids & peptides to help thicken the skin, and creams with caffeine to constrict blood vessels and reduce dark circles. You can also use cold compresses to temporarily constrict the blood vessels. Alternatively, we can do fat grafting, to inject a layer of fat between the skin & blood vessels/ underlying muscles. Fat reflects light better than vessels & muscle layer, and hence adds a luminosity to the under eye area.

Allergic and Irritant Reactions
Allergic reactions or irritant responses can lead to swelling, inflammation, and darkening of the under-eye area.
- Treatment: Managing allergies and avoiding irritants can alleviate symptoms. Cold compresses can also help reduce inflammation and dark circles. DO NOT RUB, frequent rubbing worsens the under eye darkness.
