Understanding facial ageing
Why the face sags — and why creams and fillers can only go so far
Facial ageing is not a surface problem. The changes are structural — involving muscle, fat, and bone — and only surgery addresses the underlying cause.
The face ages in three dimensions. Skin loses elasticity and thins. The SMAS — the deep muscular layer beneath the skin — descends. Fat compartments deflate and shift downward. And the underlying facial skeleton gradually resorbs, reducing the bony scaffold that supports the overlying tissues.
Non-surgical treatments work within this landscape: fillers restore volume, anti-wrinkle injections soften expression lines, threads provide temporary lift. But they cannot reposition descended muscle, remove excess skin, or correct deep folds. Each has a ceiling — and most patients eventually reach it.
A facelift surgically repositions the SMAS and removes excess skin — addressing the structural causes of ageing, not just its surface manifestations. The result is a natural, lasting rejuvenation that looks like you at a younger age, not like someone who has had surgery.
SMAS Descent
The deep muscular layer of the face descends with age — pulling the midface, jowls, and neck with it. Only surgical repositioning corrects this.
Volume Redistribution
Facial fat pads deflate and migrate downward — creating hollows above and heaviness below. A facelift repositions the tissue rather than simply adding filler volume.
Skin Excess
Skin that has lost elasticity cannot be tightened by any topical treatment. Excision and redraping is the only way to remove true skin excess.
Filler Has Limits
Repeated filler creates a 'pillow' effect over time — adding volume where repositioning is what is needed. Surgery is the structural solution.
Dr. Ankit's surgical approach
The Deep-Plane Facelift — natural rejuvenation, not a pulled look
The goal is not to look like someone who has had a facelift. The goal is to look like yourself — ten years ago. Dr. Ankit's approach achieves this through deep-plane technique.
SMAS Elevation & Repositioning
Rather than only pulling the skin, Dr. Ankit releases and repositions the SMAS — the deep fascial layer — back to its youthful position. This provides a natural, lasting lift that skin-only techniques cannot deliver.
Precise Skin Redraping
Once the deep layers are repositioned, the skin is redraped without tension — placing it back where it naturally belongs. This eliminates the tightened, 'windswept' look that results from skin-only pulling.
Neck & Jawline Contouring
Neck banding, jowls, and loss of jawline definition are addressed simultaneously — platysmal muscles are tightened and excess fat removed. The face and neck are rejuvenated as a single, unified unit.
Most patients are surprised how natural the result looks. The test is simple: people who know you notice you look refreshed — not operated-on.
Recovery is 2–3 weeks. Most of the social downtime is in the first 10 days when bruising and swelling are visible. By week three, most patients are comfortable in social settings.

Am I a candidate?
A facelift is right for you if you recognise yourself here
The best candidates are typically in their 40s–60s, in good health, and unhappy with sagging that non-surgical treatments can no longer address.
Sagging midface and jowls
The cheeks have descended, creating jowling along the jawline. The face looks heavier and older than you feel.
Deep nasolabial folds
The lines from the nose to the corners of the mouth have deepened significantly — filler no longer adequately addresses them.
Loose neck skin or banding
The neck shows loose skin, platysmal banding, or loss of jawline definition — making the face and neck look older than you are.
Non-surgical results have plateaued
You've had fillers, threads, or energy treatments — and they worked for a while. But you're now at the ceiling of what non-surgical can achieve.
You want natural results
You don't want to look 'done' — you want to look like yourself, refreshed. Dr. Ankit's approach is anatomy-led and always prioritises a natural outcome.
You're in good health
Facelift is an elective procedure best performed when general health is good and any underlying conditions are well controlled.
A facelift produces the most durable and natural facial rejuvenation available. Dr. Ankit will assess your facial anatomy, discuss your goals, and recommend the right level of procedure for your degree of ageing.

Types of facelift — matched to your age and degree of ageing
The right approach depends on your anatomy and how much change you want. Not everyone needs a full facelift.
Mini Facelift
Addresses early jowling and mild skin laxity with a shorter incision around the ear. Quicker recovery (7–10 days), excellent for patients in their early 40s.
Full (SMAS) Facelift
Complete repositioning of the deep SMAS layer and skin for moderate to significant facial ageing. The most comprehensive and longest-lasting result.
Deep-Plane Facelift
Releases the ligaments tethering the SMAS before lifting — allows more natural repositioning of the midface. Best for patients with significant nasolabial fold depth or midface descent.
Neck Lift
Focuses on the lower face and neck — tightening platysmal muscles, removing jowl fat, and excising loose neck skin. Can be standalone or combined with facelift.
Facelift + Fat Grafting
Repositioning alone can leave the face looking deflated. Adding fat grafting to the midface and temples restores youthful volume — the most natural combined result.
Non-Surgical Facelift
Thread lifts, HIFU, or filler-based lifting for patients not ready for surgery. Results are temporary (1–2 years) but involve no incisions or recovery time.

Frequently Asked Questions
Everything you need to know before your consultation.
01 What does a facelift actually correct — and what won't it fix?
A facelift (rhytidectomy) surgically corrects the structural changes of facial ageing: descended SMAS (the deep muscular layer), jowling along the jawline, deep nasolabial folds, loose neck skin, and platysmal banding in the neck. These are gravity-driven structural changes that no non-surgical treatment fully corrects. What a facelift does not directly address: volume loss (which is treated with concurrent fat grafting), surface skin quality and fine lines (addressed with laser resurfacing or skin treatments), and brow or eyelid position (addressed with brow lift or blepharoplasty). Dr. Ankit frequently combines a facelift with fat grafting for the most natural, comprehensive result.
02 What is a SMAS facelift — and how is it different from a skin-only lift?
Early facelift techniques simply pulled and excised excess skin — producing the tight, 'windswept' look associated with obvious surgery. Modern facelift surgery repositions the SMAS — the fibromuscular layer beneath the skin — back to its youthful position before the skin is redraped. Because the structural tissue is repositioned rather than the skin being pulled, the result looks natural and lasts significantly longer. Dr. Ankit performs SMAS-based facelift, which is the current standard for facial rejuvenation surgery. The skin is then draped without tension over the repositioned deep layers — eliminating the stretched, pulled appearance of older techniques.
03 How long do facelift results last?
A well-performed SMAS facelift typically lasts 8–12 years. This means the structural changes made — the SMAS repositioning, jowl correction, and neck tightening — remain evident for this period. Ageing does continue during this time, but from a higher, more youthful baseline. Most patients who had a facelift at 55 look significantly younger than they would have at 65 had they not had surgery — even after 10 years. Lifestyle factors that affect longevity include sun exposure, smoking, weight fluctuation, and skin care habits. A mini facelift, which addresses early changes, typically lasts 5–7 years.
04 What is the recovery from a facelift like?
Facelift recovery unfolds over 2–3 weeks. Day 1: surgery under general anaesthesia, one night in hospital. Days 3–5: drains and initial dressings removed. Bruising and swelling are visible but manageable at home. Week 2: sutures removed. Most bruising is fading; swelling is reducing. The majority of patients are comfortable being seen in public at Week 3 — particularly with light makeup. By Week 4–6, most patients have returned to social and professional settings normally. The final rested, natural result develops over 3 months as residual swelling fully resolves. A mini facelift has a shorter recovery — most patients are social by Day 7–10.
05 Will facelift scars be visible?
Facelift incisions are placed at the natural transitions between the ear, hairline, and scalp — specifically in the pre-auricular crease (in front of the ear), around the earlobe, and behind the ear into the posterior hairline. These are anatomically hidden locations: when healed, the scars lie at the border of two skin territories and are virtually imperceptible to anyone who isn't looking specifically for them. Scars are typically red for the first 3 months and then fade progressively over 12–18 months. With proper aftercare including silicone gel and sun protection, they become undetectable in the vast majority of cases.
06 Can a facelift be combined with other procedures?
Yes — and it frequently is. The most common combinations are: (1) Facelift + fat grafting: the lift repositions descended tissue while fat grafting restores volume lost to the midface, temples, and tear trough — a critical combination for a natural rather than 'deflated' result; (2) Facelift + neck lift: the lower face and neck are rejuvenated as a single anatomical unit — the neck lift is often performed simultaneously in the same session; (3) Facelift + blepharoplasty: addressing both the mid-to-lower face and the eye area comprehensively; (4) Facelift + brow lift: for patients with significant forehead and brow descent in addition to midface ageing. All combinations are performed under a single anaesthesia.
07 What is a mini facelift — and am I a candidate?
A mini facelift uses a shorter incision (limited to the area around the ear — not extending into the temporal or posterior hairline) to address early jowling and mild skin laxity. It is best suited for patients in their early-to-mid 40s who have begun to notice jowling and jawline looseness but do not yet have significant neck laxity or deep fold depth. Benefits: faster recovery (7–10 days vs 2–3 weeks), fewer incisions, and earlier return to social activities. Limitation: the correction is more modest and lasts a shorter time (5–7 years) than a full SMAS facelift. Dr. Ankit assesses your anatomy and recommends the appropriate level of procedure at consultation.
08 What is the cost of a facelift in Delhi?
Facelift cost at Panache depends on several factors: (1) Extent of the procedure — mini facelift vs full SMAS facelift vs deep-plane facelift involve different complexity and surgical time; (2) Whether a neck lift is included — frequently performed simultaneously, adding to surgical time; (3) Combined procedures — fat grafting, blepharoplasty, or brow lift performed in the same session affect the total cost; (4) Anaesthesia and facility — facelift requires general anaesthesia and a one-night hospital stay; (5) Post-operative care — skin management and follow-up is included in the Panache protocol. Dr. Ankit provides a complete, itemised cost breakdown at consultation after assessing your degree of facial ageing and planning the approach. There are no hidden charges.
